The 1% designation information is now part of the income statement form. This means that if you do your taxes on your own you can designate your 1% on form 07523-D which is a part of your income statement package and you don’t need to provide a separate designation slip in a closed envelope any more.

In case your employer prepares your income statement, you have to give them your 1% designation slip in a closed envelope.
